Equipment Coverage Data Forms

A completed Equipment Coverage Data Form must be submitted for each well you wish to get insured.

Well Audit Report Forms

A completed well audit form must be submitted upon the completion of an insured well

Agent Producer Agreement Instructions & Forms

As a new producer with Transpac Managers, the following actions need to be executed:

1. Complete the Producer Profile and return.
2. Review, sign, and return the Producer Agreement to Transpac Managers.
3. Complete and return a W-9 form.

In addition to these, you will also need to forward updated copies of your Agency Licensing and E & O Umbrella documentation, where applicable.
